The General Information section requests the number of copies of Form 159.

General Information

The General Information section requests the number of copies of Form 159.

How many SBA Form 159s (that is, Fee Disclosures) should be created? The default value is 1, this number can be increased if additional Form 159s are required. The number indicates how many Form 159 Fee Disclosure documents are generated, which the user can fill out for each agent who is charging for services rendered or to be rendered to the Applicant or Lender.
