Summary of Forms

There are three primary UCA forms discussed in this section.

UCA-RE

This form includes the Request for Government Monitoring section for the purposes of Regulation B (ECOA) and Regulation C (HMDA).

UCA-ADD-CON

This is a companion form to the Universal Credit Application (UCA). This document can be used for two distinct purposes. First, it can be used as a continuation or overflow page when the applicant has additional assets and liabilities that will not fit on the UCA itself. Second, another instance of this same form can be used to gather information about ?other parties? - who are not applicants, but who may be, for example, a cosigner or guarantor.

UCA-ADD-CWS

Summary

This is a companion form to the Universal Credit Application (UCA). This document is optional and can be used the creditor as a worksheet to summarize verified information about the applicants and other parties and to summarize the terms of the loan, if it is approved. Information can also be gathered to document the decisioning process.
