This is a companion form to the Universal Credit Application (UCA). This document can be used for two distinct purposes. First, it can be used as a continuation or overflow page when the applicant has additional assets and liabilities that will not fit on the UCA itself. Second, another instance of this same form can be used to gather information about ?other parties? - who are not applicants, but who may be, for example, a cosigner or guarantor.

Example common file reuse. Certain jurisdictions have a central filing system for vehicle products and use state specific forms that are filed by lenders to perfect their security interest in vehicle products.
